Q. My 11-month-old daughter has been getting blisters on her hands and feet since she was 4 months old. She gets these whenever she crawls or walks. The dermatologist says that this is due to friction of the tender skin. He says that her upper layer of the skin is very sensitive, which leads to these boils. Is there a permanent cure for this? My wife also gets blisters.

A. Your child as well as wife have epidermolysis bullosa simplex (EB Simplex). It is a genetic, inherited disorder. Diagnosis is made by histopathology, salt-split technique of immunofluorescence, electron microscopy and immunoblotting technique. Blistering in the hands and feet will improve with age but it never goes completely. Fresh blisters should be drained after puncturing them with a sterile disposable needle as they extend if left alone. The blister roof should be left in situ. Ideally, the footwear used should be made of very soft leather, with a minimum number of internal seams. Towelling-type sports socks may be worn. It is sometimes useful for the patient to wear two pairs of socks as this helps to reduce friction. It is important not to have rough internal seams in clothing and that it fits loosely, especially at the neck, wrists and ankles. Avoidance of a high environmental temperature is a helpful measure. Short courses of oral prednisolone to reduce blistering temporarily may be considered in any type of EB under the supervision of a doctor.

How To Manage Allergy Due To Xylometazone Ointment?

Q. My glans penis is hypersensitive. I therefore have the problem of premature ejaculation. I applied xylometazoline ointment on my penis for three months. I also used condoms to avoid adverse affects in my partner. I used to have sex once a week and it was very enjoyable. It was during this period that my partner had an orgasm for the first time. I also washed my penis immediately after sex. However, recently I have developed some irritation on the top of my penis. Reddish spots have developed, which are very irritating. I have applied different antifungal creams. They reduce the irritation but the problem recurs once I stop using them. The problem also recurs when I have sex. As a result I have not had sex for the past one month. Please advise.

A. The glans penis is the sensitive part of the male genitalia. If sex is initiated without stimulating the partner with pre-sex activity, it may lead to premature ejaculation. The female receptive organ may not be lubricated, hence an hurriedly initiated act leads to premature ejaculation. Regarding your problem of irritation alter topical application of xylometazoline and condom use, it could be due to allergy to xylometazoline or, though less likely, even to the condom. Oily lubricants are generally avoided with latex condoms; only liquid-based lubricants can be used. In your case, stop applying xylometazoline first. Apply Flutivate cream once daily for 7 days and then stop. This will hopefully take care of your problem. If it does not, consult a dermatologist. Sometimes, condoms do cause allergy. If the problem recurs later on after stopping topical xylometazoline, then it is probably due to the condom. In that case, condoms which do not have any fragrance, may be more suitable.

Nasal symptoms too may benefit from antihistamine treatment or nasal steroid sprays or drops used regularly. Symptoms starting after the age of 30 years are not as likely to be due to allergy as in younger patients

The diagnostic test is to apply an ice cube to the skin of the forearm for 5 minutes, and then see if a wheal develops over the next few minutes.

Q. I have skin allergy on contact with petroleum products such as kerosene, diesel, lubricating oils, etc. On coming in contact with these oils I get mild redness of the skin. I work in a petroleum refinery. However, I come In contact with oils only occasionally, maybe once in 6 months or so. I am not allergic to any medicines. Kindly advise what precautions should be taken and medicines for short-term occasional exposure to these oils. Also, please suggest a long-term solution for this problem.

A. It is not rare to get allergic reaction to oil or its constituents. As you do not get blisters or oozing lesions on your hands, I can presume that your dermatitis (eczema) is allergic and not irritant. It is to be remembered that oils most commonly produce folliculitis and acne. They may cause irritant contact dermatitis (ICD) but rarely allergic dermatitis. The factors contributing to irritation include the chemical properties of the substance; the amount and concentration of the chemical coming in contact with the skin, and the length and frequency of the exposure; and the temperature, humidity, moisture barometric pressure, friction and contamination. The remedy for your problem would lie in wearing hypoallergenic gloves for the time period that you come in contact with the oils. Occupational irritant contact dermatitis can be avoided by the following measures: personal hygiene (hand-washing); substitution with a less harmful substance; education about the harmful properties of the contact substance; wearing protective clothing (aprons, gloves with the manufacturer’s specifications, e.g. kind of material. duration of exposure! glove contact time with the chemical); and convenient washing facilities. Occupational contact dermatitis may be treated with compresses, creams, ointments and skin c1eansers .

However, one must be prepared to treat any allergic response with appropriate medications since allergy can be surprisingly variable, and a cautious approach is always needed. It is interesting that many asthmatics, for instance, grow out of their disease and become more tolerant to allergens, although other subjects may remain susceptible to allergic reactions. Perhaps the use of yoga could prove more helpful for these latter people.
